Job Description
We are currently seeking a compassionate and dedicated Bilingual Early Head Start (EHS) Case Manager II (Advocate II) for our Norristown location. In this role, the Bilingual EHS Case Manager II (Advocate II) will conduct early learning-focused and curriculum-based in-person home visits to pregnant individuals and parents of infants and toddlers. The primary objective is to empower families to enhance their health, social well-being, and their child's developmental journey towards school readiness. Responsibilities include conducting home visits, providing health education, facilitating connections to community resources, and promoting child development.
Essential Functions
Maintain all necessary credentials and certifications for the role
Plan and conduct educational home visits with families
Provide prenatal education and resources
Approach all interactions with clients and families with professionalism and cultural sensitivity
Deliver curriculum-based education sessions and resources to support family needs
Connect families to relevant community resources and social services
Conduct client assessments and collaborate with families to set goals
Coordinate health and education-related appointments
Participate in community outreach and awareness efforts
Maintain accurate records of client communication and progress
Attend regular meetings and trainings
Knowledge, Skills, And Abilities
Willingness to travel within the Greater Philadelphia or Montgomery County region
Strong understanding of community health issues, particularly relating to pregnancy and parenting
Ability to build and maintain professional relationships
Proficiency in using computer software for documentation and communication
Organizational skills with attention to detail
Experience in Early Childhood Education, Social Work, or related fields
Bilingual proficiency in English and Spanish
Valid driver's license and clean driving record
Preferred Qualifications
Bachelor's degree in a related field
Previous experience in community outreach
Knowledge of child development and early childhood education
Experience with maternal and child health issues
Familiarity with Early Head Start programs
